Como criar um formulário completo para minha rádio web

Eu quero criar um formulário completo para usar na minha rádio web. Já cliquei na opção de criar formulário e estou seguindo as etapas, mas tenho dúvidas sobre qual modelo escolher entre clássico ou cartão. Estou usando o editor Element e adicionando campos, como texto curto, mas depois de adicionar o campo idade não consigo adicionar uma nova página. Também tenho dúvidas sobre onde escrever em campos como "tipo de participação" e se seria em sub rótulo. Fechei o editor sem querer e agora não consigo encontrar o formulário que estava criando. Preciso de orientação passo a passo para criar o formulário completo do jeito certo para minha rádio web.

Now, coming back to your question, Jotform has 10,000+ customized templates that you can use. Dragging and dropping features or elements makes it really easy to create forms. I'll help you do it:

1. On your Workspace page, in the top-left corner of the page, click on Create.

2. Then, click on Form and, select Create From Scratch.

3. On the next page, select Classic form if you want to show multiple form elements on a page, and choose Card form if you want the form elements shown individually on each page of the form.

4. In Form Builder, click on Add Element in the left corner of the page to see the complete list of Form Elements, Payment Gateways, and Widgets.

5. In the menu that opens, drag and drop the Form Elements, Payment Gateway and Widgets you want to use.
