Formulário Longitudinal: Coletar respostas mensais em um único formulário

É possível criar um formulário, no qual o entrevistado precisa responder as mesmas perguntas, mês a mês?

Explicando melhor, eu quero que uma mesma organização me diga, quantas pessoas foram atendidas por um projeto social, entre os meses de abril/26 a dezembro/26. Porém, quero que estas respostas sejam concentradas em um único formulário, com as respostas sendo coletadas nos meses específicos.

Então, no mês de abril a organização me responderia o número de pessoas atendidas naquele mês. No mês de maio, o número de pessoas atendidas em maio e assim sucessivamente. Porém, quero enviar um único formulário de respostas, sendo que o preenchimento é feito nos momentos oportunos.

Now, coming back to your question, it is possible to create a form in Jotform that allows respondents to answer the same set of questions month by month. You can design a form that includes fields for each month, example, April 2026, May 2026, and so on until December 2026, where the organization can input the number of people served for each specific month.

This way, all responses are collected within one form submission, but the form can be filled out over time as the data becomes available. Alternatively, if you want the organization to submit data month by month separately but still have all data linked to one form, you can use the same form link repeatedly each month.

Each submission will be recorded individually, and you can track responses over time in Jotform Tables or Reports. To set this up effectively, you might consider adding clear instructions in the form to guide the organization on how to fill out the data for each month or how to submit the form monthly.
