Looking for a personality test template

Hola estoy buscando un formulario para medir personalidad, los usuarios contestan opciones multiples que al final tienen un puntaje que ellos no ven , la encuesta va sumando los puntos que van eligiendo de cada respuesta y al final sale los resultados que obtuvo el usuario, que por rangos , por el puntaje que obtuvo muestra la personalidad que le corresponde, tienen algun formulario que vayan contestando y les pueda muestrar al usuario sus resultados en automatico ?
