Opción se desmarca automáticamente

En la página 10, al final, aparece esta pregunta:

Quienes contestan el formulario me han levantado que al marcar una opción, ésta desaparece automáticamente y no les permite avanzar.

Agradezco que puedan orientarme en una solución.
