Form with single choice questions

Now, coming back to your question, creating a classic form is easy. Let me walk you through it:

1. On your My Workspace page, click on Create on the top-left side of the page.

2. Select Form from the assets, and then click on Start From Scratch.

3. In the next window, click on Classic Form. 

Now, we will add a Single Choice field in the form after creating it. Let me show you how:

1. In Form Builder, click on Add Element on the left side of the page.

2. In the Form Elements menu, under the Basic tab, scroll down the list and choose the Single Choice field.

3. Now, just drag and drop it to your form.

4. Click the options to modify it.
