PDF Submission: The downloaded copy still shows the old header

Editei todo fomulário com o nome Sexta Jazz, porém quando baixamos o fromulário preenchido ele vem com nome de Domingo Com Teatro, não estou achando onde aditar este nome, pois aparentemente eu editei todos os títulos. Podem me ajudar?

I reviewed the last form you've been working on, and it appears that you're referring to the PDF submission copy where the name 'Domingo Com Teatro' was still visible. If you receive an outdated PDF submission copy, you can quickly resolve it by generating a new PDF document and removing all previous versions. Making a new one will update the content. Updating the PDF in your form is easy. Here’s how to do it:

• Go to your PDF Editor, or click this link to go directly there.
• To create a new one, click + New PDF in the top left corner. Then, choose New PDF Document. 
• You can also delete the previous one by clicking the three dots next to the PDF name. Then, from the list, select Delete.
