Form prefill: create button not working

Bom dia,

Estou tentando criar um filtro de pré-preenchimento de um formulário e o botão "criar" não está funcionando, ele não executa nenhuma ação quando clica nele. Até ontem esse recurso estava funcionando perfeitamente... tentei excluir alguns pré-preenchimentos utilizados anteriormente, para verificar se não era por conta da quantidade, mas o erro permaneceu.
