Thanks for the feedback, the new URL is working, but why did this happen, is Jotform changing the form links?

I would like to know, because from what I observed I have other forms with the old "format" of the links

Obrigada pelo retorno. Verifiquei o link utilizado e também pude observar o problema. No entanto, realizei a limpeza de cache dos teus formulários para eliminar quaisquer questões relacionadas e isso parece ter resolvido o problema. Já consigo acessar o teu formulário em ambos os links. Você pode conferir se o formulário está funcionando normalmente do teu lado? Caso continue precisando de ajuda, envie-nos uma nova mensagem.
