I would like to ask for help with another thing. In "Checklist de Ferramentas", I created conditions which as you select the "Unidade" (that means the branch), it opens the employees of that branch. Let's suppose I did all this things and choose the wrong employee, in that case I need changing the branch e select other employee name. I believed the checklist would disapear and it shows the new one, but thats not what happen, it kept last and open a new one above. Is that anything we can do to change it? As I select a new name, the last checklist disappears.

Follows the example in the link bellow:

    Hi Calebe,

Thanks for reaching out to Jotform Support. The condition to show the individual checklist is only based on the dropdown field that selects the name of the employee. But if you include the corresponding branch into the condition, the checklist will disappear when you change the branch. Here's a sample condition applied to the Belford Roxo branch:

If a different branch is selected, the checklist will disappear allowing the user to select another employee to view its checklist. Here's a screencast showing how it works:

Give it a go and let us know if anything else comes up.
