Now, let me help you with your question. I checked your account and can see here that it was suspended due to collecting log in credentials, which is a violation against our  Terms of Use. To reinstate your account, we will need your permission to remove the offending fields from your form.

Once we hear back from you, we'll be able to move forward with a solution.
