Thanks for reaching out to us for help. Adding a review widget is easy. Let me show you how:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Build.
2. Click on the Add Form Element menu on the left side of the page.
3. Click on widget section. Select Review Before Submit.
