Now, coming back to your question, it looks like your account was suspended due to one of your forms (Form ID: 252745765972068) containing fields to gather payment information. Collection of payment information in plain form fields isn't allowed and instead must be gathered via our payment processors instead.

I've gone ahead and deleted these plain form fields and reactivated your account.

In the future, you will need to use a payment widget.
