NNathanI am experiencing an issue where my account appears to be suspended. The message displayed states: "Account Suspended. Your account has been suspended because our automated system detected activity that violated our Terms of Use." I want to reactivate my account but the process is not working. I have provided my details multiple times, including my username "nherve570" and my email "Nherve570@gmail.com." I have asked what caused the suspension and which form blocked my account, but I have not received a clear explanation or resolution. I am frustrated because the reactivation attempts are unsuccessful and I need my account unlocked.

When I checked your account, I noticed that it was auto-suspended by our system because your form titled Formulaire includes fields to collect sensitive information, which violates our Terms of Use. I’ve removed the offending fields and reactivated your account.
In addition, we'll need you to send the following information to reviewers@jotform.com with Thread #33053691 as the subject:
- The URL where the form is used.
- A link to your website or proof of affiliation with a legitimate institution.
- Any supporting documentation that can validate your claims.
- A valid government-issued ID card, such as a driver's license or passport, along with a clear, forward-facing selfie for verification.
Once we hear back from you, we'll be able to help you with this.
