For my form "Demande d'inscription AMT 2027 - USA & Canada", I'm having trouble setting up all the conditions:
1. "Y a-t-il un passager ?" (Is there a passenger?)- If No → "Date de naissance passager moto" should not appear.
- If Yes → it should appear, for both Moto and Auto.
- If Yes → "Nombre de passagers / participants" should appear only with Auto.
2. "Nombre de passagers / participants"
- Should appear with all Auto options, and depending on the selected number (1 to 4), display the matching birth-date fields.
3. "Date de naissance passager 1" to "Date de naissance passager 4"
- Should not appear with Moto.
- according to the number of passagers
4. "Date de naissance passager moto"
- Should appear only with Moto.
5. "Passager 1 " to "Passager 4"
Should appear only with Auto, and according to the number of passengers selected.
6. Cellphone field
- I would like French (+33) to be pre-selected, so users don't have to scroll to find France.
I've reviewed my conditions and tried changing them several times, but it still doesn't work.

1. Base setup so fields are hidden by default
- Go to your form in Form Builder.
- Select each of these fields and set them to be hidden by default using their properties: Nombre de passagers / participants, all Date de naissance passager 1–4, Date de naissance passager moto, and the name fields Passager 1–4.
- Keep visible only the main questions: vehicle type (Moto/Auto), Y a-t-il un passager ?, and Nombre de passagers / participants (the last one will be shown by conditions).

- AND IF Type de véhicule Is Equal To Moto
- DO > Show > Date de naissance passager moto
- Add another condition so that the Auto passenger block appears only when there is a passenger and Auto is selected, for example:
- IF Y a-t-il un passager ? Is Equal To Oui
- AND IF Type de véhicule Is Equal To Auto
- DO > Show Multiple > select Nombre de passagers / participants and (optionally) the Auto passenger name/date fields you want to control together.
This way:
• If “Non” → no passenger fields are shown.
• If “Oui” + Moto → only Date de naissance passager moto appears.
• If “Oui” + Auto → Nombre de passagers / participants appears (moto fields stay hidden).3. Conditions for “Nombre de passagers / participants” (Auto only)
Make sure your dropdown has clear values like 1, 2, 3, 4. Then create one condition per value to show the correct set of fields.
- For 1 passenger (Auto):
- IF Type de véhicule Is Equal To Auto
- AND IF Nombre de passagers / participants Is Equal To 1
- DO > Show Multiple > select Passager 1 and Date de naissance passager 1.
- For 2 passengers (Auto):
- IF Type de véhicule Is Equal To Auto
- AND IF Nombre de passagers / participants Is Equal To 2
- DO > Show Multiple > select Passager 1–2 and Date de naissance passager 1–2.
- Repeat the same logic for 3 and 4 passengers, always including only the corresponding Passager X and Date de naissance passager X fields.
Because every one of these conditions includes Type de véhicule = Auto, the fields Passager 1–4 and Date de naissance passager 1–4 will never appear when Moto is selected.
4. Ensuring Moto vs Auto separation
- Add a condition that hides all Auto passenger fields when Moto is selected, for extra safety, for example:
- IF Type de véhicule Is Equal To Moto
- DO > Hide Multiple > select Nombre de passagers / participants, Passager 1–4, and Date de naissance passager 1–4.
- Add a condition that hides the moto passenger date when Auto is selected:
- IF Type de véhicule Is Equal To Auto
- DO > Hide > Date de naissance passager moto.
- Test in Preview mode by switching between Moto/Auto and changing the number of passengers to confirm that only the correct fields appear.
5. Cellphone field with France (+33) pre-selected
If you are using one of the phone widgets that supports a default country, you can preselect France so users don’t have to scroll.
- Click on Add Element and go to the Widgets tab.
- Use a phone widget such as International Telephone Picker, Phone Picker, or Country Code (depending on which layout you prefer) and add it to your form.
- Open the widget’s settings (gear icon), and under the General tab, set Default Country to France (+33), then click Update.
If you prefer to keep your existing phone field and it does not have a “Default Country” option, you would need to replace it with one of these widgets to have France pre-selected.
