Now, coming back to your question, Can you share with us the form URL in question and the custom URL name you are trying to create? Can you let us know what type of the error are you getting when you are trying to create a custom form URL? Using your domain in your form link, like the one below, is only available with Jotform Enterprise:

https://yourcompanydomain.com/formId

But, you can add a Custom URL to your form. Here’s how to set it up:

1. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.
2. Next, click on the Pencil icon under the Settings button. 
3. Edit your form link, and click on Apply.

Now, your form link will look like the one below:

https://form.jotform.com/yourusername/yourformname

Can you share with us a screencast while you are creating a custom form URL? You can also test it and share with us the screencast using Loom or Google Drive 's public link so, we can access it. I've cleared your form caches; your forms are now refreshed. Can you clear your web-browser caches and then try again using Google Chrome's incognito mode?
