One of my form is not working

On ma form called " reservation" I have a problem for one month. It does not go. Others works properly

Could you help please and see what's wrong, I can't

If answer available in french it could be better ( as you can see :-)  )

I understood what you said Google works pretty well. We can continue in any
of our 2 langages if you understand me too.
I dont receive Emails when customers submit reservation form. Trying again
today by myself from website, I got a white sheet with address;

From the meantime this problem appeared I have another one with Paypal
payment form but I think this is from Paypal and not from your system.

Your form works well here on my end. I was able to submit and go to this Thank You message:

If your form is embedded on a webpage, please tell us the URL so we can test the form for inconsistencies.
