Now, coming back to your question, unfortunately, we didn't get the link to the video. Can you copy and paste the link here? You can also share the video recording on any public sharing platform like Google Drive or Dropbox, or you can simply use Loom to record the video and share the video link with us.
Please also share the link to the form where the issue is happening.

I can see that there's a logo uploaded to your form. Take a look at the screenshot below:
Can you try to use a different web browser? You may also try to restart your internet modem or connect to a different internet service provider.

I'm glad the issue is fixed. As I can see on your video, the Jotform servers returned an error 429, which may to come from your browser or your connection to the internet which send many connection requests. The same reason made the CAPTCHA displaying to verify if you are human. Your computer may have sent many clicks at the same time, or a browser extension was interfering, or something was wrong in your connection to the internet.
