When I cloned and tested your form, the form copilot worked without the permission access error. Check out the screenshot below to see my results:

I've gone ahead and cleared your form caches. I would also suggest clearing your browser's cache as an additional aid. Can you try it again and see how it goes? If the same thing happens, can you confirm whether the error appears every time you make a request in Form Copilot, or does it show immediately when opening Form Copilot before making any requests?
A screen recording would help us see exactly what’s happening. You can use Loom or upload to Google Drive/Dropbox, then send us the link.
