I checked your form, and it seems that the conditions you've set up is causing the Shipping Address field to be hidden. If you don't need these conditions, you can delete or disable them. Let me show you how:

1. In Form Builder, click on Settings in the orange navigation bar at the top.
2. Select Conditions on the left.
3. Select all the conditions, and click on disable. (You may also delete them if you don't need these conditions)
