Can you please check your account's Trash to see if it's there? Let me show you how:

1. On the left side of your account's My Forms, scroll down to reach Trash and click on it.
2. Hover your mouse to the form you're looking for and once found, click Restore on the right. Go back to All Forms to see the restored form.

Please note that there is an orange banner on the top portion of the page. That indicates that a deleted form will be retained for 30 days before it's fully deleted or purged. Additionally, if you are unable to find the form, unfortunately there is no way it can be restored any more.
