Can not submit form

I have filled out the form completely. When I click on the "Check answers" button, I am taken to the beginning of the form. Although all mandatory fields are filled in, it is not possible to submit.

It looks like this field is not filled in completely. Please try filling that in and seeing if you can submit.

I was unable to replicate this when making a clone of the form. If you're still having problems I would suggest contacting the form owner as we can't try doing things to their form without their permission.
