Hello,
I would like to create a health quiz with the following setup:
• 30 questions across 5 categories (systems)
• Each question has 3 answer options with point values: Rarely = 0, Sometimes = 1, Often = 2
• The points are calculated separately per category (6 questions × max. 2 points = max. 12 points per system)
• After submitting, the user should automatically receive a personalized email based on which system scored highest
• If two systems both score 7 or more points, a specific combination email should be sent instead
My questions:
1. Can Jotform calculate scores separately per category/section?
2. Can I set up conditional email notifications based on which category has the highest score?
3. Can I handle the combination logic (e.g. System 1 ≥ 7 AND System 2 ≥ 7 → send a specific email)?
4. Is this possible on the free plan, or which plan do I need?
Thank you very much!

Podo: Support SpecialistHi Natalia,
Thanks for reaching out to Jotform Support. I'm an AI assistant here to assist you.
Let me help you with that:
1. Separate scores per category/section
Jotform can create quizzes and calculate scores using its quiz features (Quizform / Online Quiz Maker). However, the available information only confirms that Quizform compares answers to a set of correct answers and produces an overall result. It does not describe a way to calculate and store separate numeric scores per category (e.g., 5 systems each with its own subtotal) in the way you outlined. Because this behavior is not documented, we cannot confirm that your exact per-category scoring logic is supported.2. Conditional emails based on which category has the highest score
The documented email logic is based on field answers/conditions, not on “highest score among categories.” You can:- Set up multiple email templates (Notification or Autoresponder).
- Use Change E-mail Recipient conditions so that specific emails are sent when certain answers or conditions are met.
However, there is no documented feature that automatically detects “which category has the highest score” and then uses that as a condition. Conditions can react to field values, but the knowledge does not confirm a built-in “max of several scores” comparison.
3. Combination logic (System 1 ≥ 7 AND System 2 ≥ 7 → specific email)
The available information shows that you can:- Create multiple email templates.
- Use Change E-mail Recipient conditional logic to send different emails based on specific answers.
But the documentation does not explicitly describe numeric comparison conditions like “field ≥ 7” combined with another numeric condition “AND other field ≥ 7” for sending a specific email. Since this exact type of numeric, multi-field logic is not detailed, we cannot confirm that your combination rule (System 1 ≥ 7 AND System 2 ≥ 7) can be implemented as described.
4. Plan / free plan limitations
For conditional emails, there is a clear documented limit:- Starter (free) plan: Up to 3 Change E-mail Recipient conditions per form.
- Paid plans: Unlimited Change E-mail Recipient conditions.
So, if your logic requires more than 3 separate conditional email rules (for multiple systems and combinations), you would need a paid plan to go beyond that limit. The information does not specify any other plan-based restriction directly related to quiz scoring itself.
In summary, Jotform can create quizzes and send conditional emails based on answers, and the free plan is limited to 3 conditional email rules. However, the exact advanced scoring behavior you described (separate per-category scores, picking the highest system, and complex numeric AND-combinations for emails) is not documented, so it cannot be confirmed as supported from the available information.
Let us know if you have any other questions.

Keenly Jotform SupportHi Natalia,
While our AI assistant shared some information, let me also provide some details about your questions. Here are the quick answers:
- It’s possible to assign point values to options in form elements such as Single Choice or Multiple Choice fields using our Calculation Values feature.
- Use our Form Calculation widget to set up more complex calculations based on the selections made in the different sections of your form.
- You can change the email recipients or the email itself based on one or more rules using the Change Email Recipient condition.
Reach out again if you have any other questions.
