Even small changes, like editing column headers, changing the file name, or adding/removing rows, can cause the widget to stop matching values.

Let’s try the quickest fix:

1. Download your latest spreadsheet.
2. Make sure the first row contains the original column headers exactly as they appear in your form fields.
3. Re-upload the spreadsheet into the Spreadsheet to Form widget.
