Now, coming back to your question, it is possible with 2 Date Field element and 1 Number element. Let me walk you through the steps:

Adding the necessary elements:

1. In Form Builder, click on the Add Element+ menu on the left side of the page.
2. Under Basic tab, drag-and-drop Date Picker and Number element to your form.
3. Click on the second Date Picker, then click on gear icon.
4. On Date Picker setting, click on Option tab.
5. Under Default Date, set the settings to Current.

Adding the calculation:

1. In Form Builder, click on Settings on the top orange navigation bar.
2. Click on Conditions on the left sidebar.
3. Select Update/Calculate Field.
4. Add the following conditions:

If you want to prevent the number 55 to show, editing the Condition you created to calculate the age. Doing that is easy. Let me show you how:

1. In Form Builder, click on the Date Picker field 本日の年月日, then click on the Conditions icon on its right side.
2. Howe with your mouse on the condition and then click on the Pencil icon.
3. Set the condition as displayed in the screenshot below, then click on the Save button:
