Suspended account: Phishing

    Now, coming back to your question, I've checked your account, and it is currently suspended because your form is suspected of Phishing. To reinstate your account, please provide the following information:

    1. The URL where the form is used.
    2. Provide any documentation that will support your claims/legitimacy.
    3. The website or proof of affiliation to legitimate institutions.

    Send all of this information to reviewers@jotform.com with Thread #14317943 as the email subject. One of our colleagues from the Reviewer team will get back to you about the status of your suspended account.
