J'aimerai créer un formulaire comme dans le lien ci-dessous ci-dessous avec le même design mais le plus important en fonction des réponses du client générer en format PDF  bulletin d'adhésion qu'il sera transmit automatiquement par mail avec moi en copie.

Yes it is possible to create similar form in JotForm. Please go ahead and build your form with desired form field and we will help you with custom css code to customize the form to match the look and feel to make it similar to the form you have shared.

As far as I understand, you need to create a table which would have multiple columns with static text and 1 column with radio buttons. I'm afraid, we do not have an element to meet this requirement out of the box, but I think you can check our Input Table or Configurable List widget:

I can not find a solution for a box like in my request of 07, 2019 at 08:36 AM, can you find me a solution or do a real search for a solution, I find it surprising that you do not have something of similar, I tried to personalize your table without success

As mentioned earlier, there is no build in field or widget to achieve your requirement. I will try few workaround and will get back to you as soon as I am able to achieve something similar what you have shared in screenshot.

Unfortunately, I also tried to find an alternative for this, but I couldn't find any. I have created a feature request for this but note that we cannot give any estimated timeframe.

Also note that we cannot guarantee if this will be implemented since it will depend on the number of users requesting for such feature.

To my understanding, you would like to have the table in the form that my colleague created to be added to the form you linked in your last reply, is that correct?

I'm not sure if the table will fit in the form you linked since the form is too narrow for the table.

May I suggest that you create a cloned version of my colleague's form first to your account?

You can recreate the table by adding a Text Element on the form you just linked. Then, copy what is in the source code of the Table in the cloned version of my colleague's form.

You will also need to recreate the Single Choice and Multiple Choice fields in the cloned version.

Lastly, copy the custom CSS codes that my colleague injected in the cloned version to your the form you linked.

I am unclear regarding your question, can you send me the code?
I see you are currently over your  Form limit off 25 for the Bronze subscription. In order to create more forms, you would need to delete some forms or to upgrade your account.

you gave me the table additional guarantees

Please note that just adding the content of the text field will not be sufficient to get the full functionality of my demo form. There are several other fields and custom css code injected in form to achieve your requirement. 

The best way forward would be cloning the form in your account and then customize it based on your requirement. Are you using all forms you have in your account? Please delete the forms which is not being used and then clone the demo form I shared with you.

Since my colleague's form requires multiple changes to be able to fit to your form, allow me some time to integrate it to your form properly. Will get back to you once done.

I was able to make the necessary adjustments to your form so the radio button fields and the checkboxes are aligned under the last column of the table, which was created using a Text element.
