Alternatively, if you plan on making your Canva design offline, you can add a form QR code to your Canva Design in just a few simple steps. Let me show you how:

1. Open your design in Canva, and then hover your mouse over the panel on the left and click on Apps.
2. In the menu that opens, search for QR Code and click on it. 
3. Enter the URL link of your form and click on Generate code.
