    Thanks for reaching out to Jotform Support. I'd like to let you know that I am using a translation tool to provide assistance, but if you would prefer to get assistance in the Dutch language, we can forward your ticket to our Dutch team. However, please note that it could take longer to receive a response.

    We have a known issue where the form with the link "form.jotform.com" will not work because it has been blocked by some Internet Service Providers (ISP) in Belgium. Our developers are aware of this and they are now working on fixing the issue. We will circle back on this thread once we hear from them.

    In the meantime, you can remove "form" from the link and change it to "eu". Here's a sample link: https://eu.jotform.com/{form-id}

    It would also be helpful if you can also contact your provider for IP blacklist removal.

    Thanks for the info. Just changed the "form" to "eu" and it seems to work.

    Thanks for getting back to us. We'll notify you when the issue will be resolved by our developers. In the meantime, as already suggested by my colleague, you can contact your ISP asking to remove the domain from their blacklist, this would speed up to process.
