    Now, let me help you with your question. I'm sorry you are having issues with this. We have a known issue where the form with the link "form.jotform.com" will not work because it has been blocked by some Internet Service Providers (ISPs) in Belgium.

    Our developers are aware of this, and they are now working on fixing the issue. Rest assured that we’ll get back to you with an update as soon as we have one. In the meantime, please replace the prefill link with the following:

    It would also be helpful if you can also contact your provider for IP blacklist removal.

    We are currently experiencing a similar issue (here too in Belgium). About 40% of our customers are indeed unable to use the form.

    We are patiently waiting and hoping for good news soon!
