Automatische invoer van patiëntgegevens.

Ik ben nog even zoekende in het programma. Wij willen graag een formulier opmaken waarbij we als organisatie kunnen digitaliseren. Op die manier zouden we onze patiëntenfiches graag online willen invullen ipv momenteel op papier. Ik heb al eens gekeken om hiervoor een formulier aan te maken. Is er ook een toepassing binnen uw software om de velden met de patiëntengegevens automatisch te laten invullen bij het inlezen van hun identiteitskaart? Wij werken hiervoor met Mycarenet.

Now, I'll help you with your question. I suggest using the Spreadsheet to Form Widget. if you want to autofill other fields with data once entered, you can use this widget to autofill your client's data. Let me show you how:

First, create a spreadsheet containing the data as shown below. It is important that the column names in the spreadsheet and the field names in the form are the same.

1.  In Form Builder, click on the Add Form Element + menu on the left side of the page.
2.  Under the Widgets tab, search for the Spreadsheet to Form widget and add it to your form.
3. In the Widget Settings that open, upload the Excel file.
4.  Then, click on Update Widget.

And when you enter it there, it will automatically reflect your email:

Also, If you are looking for about passing data from one acquisition form to another, this is possible and can be accomplished automatically using the prepopulate URL method. There is also a feature called Jotform Prefill. Jotform Prefill is also available in HIPAA accounts. You can create free prefilled forms that can be connected to CRM, Jotform Tables, and previous submissions to create pre-populated forms with unique URLs.
