契約書の締結

セントケア・ホールディング(株)の藤森と申します。

貴社のサービスを利用させて頂くにあたり、個人情報の取り扱いの観点から、弊社法務部門より「個人情報の取扱いの委託に関する契約書」の締結を求められております。

お手数をおかけしますが、上記のような契約書の締結が可能なものか、ご教授頂けますでしょうか。

上記、ご認識の通り弊社の法務側にて作成した契約書へのサインのイメージとなります。

弊社が介護業界の企業でありますことから、個人情報保護の観点で契約書の締結を法務より求められております。イメージとなり恐縮ですが、契約書の内容を添付させて頂きます。

また、プランによって契約書の締結が可能等ございましたら、ご教授下さいませ。

I understand that you will be collecting personal information. Does it come with health-related data? If so, you need to have HIPAA compliance in order to collect Personal Health information.
