Now, coming back to your question, the forms are responsive by default. I tested your form to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:

Is the form embedded on a website? If it is, could you please share the link to your website where your form is embedded? I can check it out and see what’s going on. Can you take a screenshot of what’s happening when you view the form on mobile and send it to us? We’ve got a guide here that’ll show you how to do that.

I checked the website link on my end, and it seems that it works properly when viewed on my computer. Check out the image below:

As you can see, it just occupies the space needed for the form and for the navigation at the bottom, including the pagination and the Save button. Are you able to check it on your end as well? We couldn't be able to modify the form as well because you used the Card form when building your form. If you want to modify the form layout, you need to convert it into a Classic Layout.
