How to fill out a 4506-C form: A step-by-step guide to tax transcript requests

The following suggestions are for informational purposes only. For current Form 4506-C and IVES requirements, see the IRS guidance on IVES for taxpayers.

When you apply for a mortgage, loan, or another service that requires income verification, a lender may ask you to authorize access to your IRS tax transcript. Form 4506-C is the consent form commonly used in the IRS Income Verification Express Service (IVES) process to make that request.

It is important to understand what you are authorizing: Form 4506-C does not give a lender a copy of every tax document you have ever filed. It authorizes an eligible third party to request specific transcript information for the tax periods and form types shown on the completed request.

What is Form 4506-C?

Form 4506-C, IVES Request for Transcript of Tax Return, is used through an authorized IVES participant to request tax-return or wage-and-income transcript information from the IRS with the taxpayer’s consent. IVES is often used by banks, credit unions, mortgage lenders, and other participating companies during income-verification processes.

Who needs to fill out Form 4506-C?

You may be asked to complete Form 4506-C when a participating lender or service provider needs to verify your income, tax return, or wage information as part of an application. Individual and business taxpayers can use the process when the request comes through an authorized IVES participant.

The taxpayer must review the request and give valid consent before the IRS provides tax records to the designated recipient. If you only need your own transcript, consider the IRS’s direct tax-record options instead of using Form 4506-C through a third party.

What to gather before completing the form

  • Your name and taxpayer identification number exactly as shown on the tax return
  • Your current address and the address shown on the last  return filed if it differs
  • The tax form number filed, such as Form 1040, 1065, or 1120
  • The requested tax-year or period ending dates in the required format
  • The IVES participant’s and client company’s contact information, plus the customer file number or unique identifier when applicable

How to fill out Form 4506-C

1. Enter taxpayer information on lines 1–4

Enter the taxpayer’s name and TIN on line 1. If the transcript request relates to a joint return, follow the form instructions for the second taxpayer’s information. Use lines 3 and 4 to provide the current address and, when different, the previous address shown on the return filed. For businesses, verify the legal business name, address, and EIN.

If the current address and the address shown on the requested lated return filed differ and you have not updated your address with the IRS, the form instructions direct you to file Form 8822 or Form 8822-B with Form 4506-C.

2. Identify the designated recipient on line 5

Line 5 identifies the parties involved in the request. Line 5a lists the IVES participant, while line 5d identifies the client company that will receive the requested transcript from the IVES participant. Review both carefully before signing. Line 5b may include a customer file number, and line 5c may include a unique identifier, to help match the request to the application.

3. Request the right transcript on line 6

On line 6, enter only one tax form number, such as Form 1040, Form 1065, or Form 1120, and select the requested transcript type: return transcript, account transcript, or record of account. Enter the tax-year or period ending dates exactly as the form requires. Do not substitute a different form number or write a generic description such as ‘all taxes.’

4. Complete wage and income requests when applicable

The form may be used to request wage and income transcript information. Read the available choices and dates carefully; lenders may ask for specific years, income sources, or transcript types. Only authorize the data requested for the stated purpose.

5. Review, sign, and date the request

Do not sign Form 4506-C unless all applicable lines are complete and accurate. The signer must have authority to act for the taxpayer; business, trust, estate, and entity requests have specific authorized-signatory rules. The IRS form states that it must be received within 120 days of the signature date, so do not sign it too early.

How Form 4506-C works with IVES

With your consent, the lender or IVES participant submits the request to the IRS online or by fax. The IRS provides transcript information to the authorized participant through its secure process. You may be able to review and approve or reject a request through an IRS online account or business tax account, depending on the request and available options.

Common Form 4506-C mistakes to avoid

A transcript request can be delayed or rejected when the information does not match IRS records or the consent is incomplete. Review every field carefully before signing, especially the recipient and tax-period details.

  • Signing an incomplete form: Do not sign Form 4506-C until all applicable lines, including the requested transcript type, tax periods, and recipient information, are complete.
  • Using information that does not match IRS records: Enter the taxpayer name, TIN, and current and, if different, the address shown on the last return filed. For a business request, verify the legal name and EIN.
  • Requesting the wrong transcript or tax period: Confirm the tax form number, transcript type, and year or period-ending dates with the lender or service provider before authorizing the request.
  • Overlooking who receives the transcript: Review the IVES participant and client-company details on line 5. These fields identify the parties authorized to receive your transcript information.
  • Signing too early: The IRS must receive Form 4506-C within 120 days of the signature date. Sign only when the request is ready to be submitted.
  • Using an unauthorized signer: For business, estate, trust, and other entity requests, make sure the signer has the authority required by the form. An authorized representative may need supporting authorization documentation.
