FAQ
What is Form 4506-C?
Form 4506-C, IVES Request for Transcript of Tax Return, lets you authorize a participant in the IRS Income Verification Express Service (IVES) to get your tax transcripts. IVES participants are usually mortgage lenders, banks, and other companies that need to verify income.
The transcripts go to the IVES participant and client company listed on the form, not to you. If you need copies of your own transcripts, the IRS Get Transcript service or Form 4506-T is the better fit.
Who sends Form 4506-C to the IRS?
You fill out and sign the form, then give it to the lender or company that asked for it. The IVES participant sends it to the IRS through its assigned IVES process, so you don't mail it yourself.
How long is a signed Form 4506-C good for?
The IRS must receive Form 4506-C within 120 days of the date you sign it, or the request will be rejected. Don't sign until every applicable line is complete, including the transcript type and the years requested, because incomplete or hard-to-read forms can also be rejected.
What transcripts can be requested on Form 4506-C?
Line 6 covers a return transcript, an account transcript, or a record of account for one tax form number, such as Form 1040 or Form 1120. Line 7 requests wage and income transcripts, which pull data from information returns like Forms W-2 and 1099. Line 8 lists the years or periods, entered as the ending date of each one.
How do I fill out Form 4506-C?
Enter your name, taxpayer identification number, and current address on lines 1 through 3. If you filed jointly and transcripts are needed for both of you, add your spouse's details on line 2. Use line 4 only if the address on your last return is different from your current one.
Line 5 identifies the IVES participant and the client company that will receive the transcripts, and your lender will usually provide these details. Then complete lines 6 through 8, sign and date the form, and check the box confirming you have the authority to sign.
Is it safe to fill out Form 4506-C online with Jotform?
Yes. Your entries are protected with 256-bit SSL encryption, and you can download the finished PDF right away. Jotform doesn't send the form to the IRS or your lender, so you decide who gets it.
