About
Family First Health is a federally qualified health center based in Pennsylvania, delivering essential medical and dental services to underserved communities. Its school-based mobile dental program provides screenings, cleanings, and preventative care to children across multiple school districts.
Challenge
Family First Health operates a mobile dental program that visits more than 50 schools across 16 districts each year, often serving over 1,000 students in a single district. Before adopting Jotform Enterprise, the program relied entirely on paper-based consent, enrollment, and insurance forms.
Solution
Seeking to digitize its processes, the Family First Health team evaluated Microsoft Forms and DocuSign. However, Microsoft Forms did not meet HIPAA compliance or mobile usability requirements, while DocuSign lacked the customization needed for their workflows. The team selected Jotform Enterprise for its secure, customizable forms and ease of deployment.
Results
The new digital consent form went live in early August. Within the first month, Family First Health received more than 500 submissions, with approximately 25% including uploaded insurance cards, something the team had never been able to collect efficiently before.
