To create a report:
- Go to the Jotform Abuse Report page.
- Scroll down to the report form and provide the following:
- Your Full Name
- Your Email Address
- Link of the form you are reporting
- Your reason for reporting the form.
- Submit the form.
You should receive a confirmation in the form itself, and you will also receive an email copy of your report.

Il y a environ 2 à 3 semaines, je me suis inscrite sur la plateforme Singulart pour vendre mes œuvres. J’ai ensuite reçu un message par courriel d’une personne nommée Francis, m’informant qu’il était intéressé par une de mes œuvres et qu’il comptait l’acheter via Singulart.
Quelques jours plus tard, j’ai reçu un autre message indiquant :
« Bonjour, comme convenu nous venons d’effectuer le paiement via Singulart ! Avez-vous reçu leurs emails ? »
Puis, juste après, un nouveau message est arrivé, disant :
« Bonjour… Ce paiement de xxxx EUR a bien été validé par Singulart ! Pour aller le recevoir, appuyez ici : Validez ici. »
Par réflexe, j’ai malheureusement cliqué sur le lien et commencé à remplir le formulaire demandé. Au moment d’entrer mon numéro de carte de crédit, je me suis rendu compte que ce n’était pas normal. J’ai donc arrêté et je n’ai pas validé ni envoyé le formulaire.
Cependant, depuis, je ne parviens pas à fermer ou effacer la page du formulaire sur mon téléphone. Elle réapparaît chaque fois que j’ouvre un message ou un document, comme si elle restait en arrière-plan.
Pouvez-vous me dire comment l’éliminer de mon téléphone ? Est-ce un virus qui pourrait copier ou enregistrer mes mots de passe ?

I was somehow directed to your site when I was trying to sign a consent form for a Televisit for my Dr. I made a password and found a consent form filled it out and submitted it I believe. I called my Dr office and she told me their office was not associated with your company. I need to remove my information and close the account please advise me as to what I need to do to stop this. A James Daniel sent me a text says i was approved for a loan with Lending Tree. He sent me a jotform to fill out my Bank information, which I did because I was looking for a loan online so I thought it was a legit aporoval. My bank locked me out of my mobile banking. Do I need to change my bank account number and password?

I recieved a call from 7076355890, a man called Leo, that I have been approved for a Loan, and he sent me this link below.
The link was a form under Lending Tree. It asked for my Bank of America username and password! I refused and he cut the call!
This is a fraud using your company name Please let me know what to do now!

Apparently, this account was opened back in 2018. I have a screenshot of the details and the phone number is incorrect. Since it’s been so long, I don’t remember if I miss typed my phone number or what happened. Anyway, I have not used this application for years, then suddenly this morning I have two requests for a security codes that I did not request.

Im pretty sure I was just scammed. A potential employer (supposedly) texted me asking me to fill out all this info on jotform. I did and then they sent me an id.me link to fill that out as well. I didn\'t fill out the id.me, but I did fill out the jotform. My ID, lots of personal info. I have no way to stop whoever it was form getting my information. Help !
