No fields are showing up when trying to map fields in MailChimp Integration

Trying to integrate jot forms with my mail chimp

but then when it comes to

I have nothing to select in order to complete

When you say nothing, you mean not even the name and email are showing up on the integration? You have five forms. Which one here are you trying to integrate with MailChimp?  I just tried the integration, I was able to map the fields. 

I think there is an error in the console log, would you mind checking that as well? Please check the following articles on how to check this log:
