why submit button not appear?

I cloned your form (http://www.jotform.me/form/70991488867478) and I noticed that the submit button has conditions and it seems that there were missing fields on your form.

Kindly fix the conditions on your form. Or if the fields are deleted, you can delete the submit button and re-add it.
