I added a form to my Facebook page and it cannot be loaded now

I just did this, got a message that it successfully added to my page, but now when I go to my page, I get an error message that the page cannot be loaded. I can't remove it if I can't see it. It seems like it broke my page! www.facebook.com/AITSsouthlake

Please note that the Facebook page that you kindly provided, is working properly for me, and I was able to click on "Contact Us" from there, which led me correctly to your embedded form:
