Google spreadsheet is not receiving data except Submission date

experiencing exact same problem as AnkitPatni, getting all data EXCEPT submission data. Was working fine and then broke about a week ago...

Please let us know if you have made any modifications to integrated spreadsheet. If there are any modifications made to the spreadsheet, the integration would break and cannot send the submission data to the form. Could you try to reintegrate Google spreadsheet to your form and see if that helps? If the issue still persists, please get back to us with the Form ID/URL so that we can assist you further.
