Form Error when submitting

I need assistance with an issue I'm encountering on a new form I've created.

I was testing it. When attempting to submit the form, the page begins to load but eventually results in an error. I have attached a screenshot of the error for your reference.

Could you please look into this and provide guidance on how to resolve the issue?
