    Hi Vamsi,

    Thanks for reaching out to Jotform Support. I took a look at your account, and it was suspended because your form with ID #241331764316048 collects login credentials. Collection of these data like Password, Security Q&A and Phrases, and PIN codes aren't allowed regardless of the type of the account. To activate your account, we need to delete the offending fields, and please refrain from collecting login credentials; it is a violation of our Terms of Use.

    Subject: Request for Amendment of Form Fields and Account Reinstatement.

    My name is Vamsi Krishna, and I am writing to you formally to address an issue concerning certain fields in a form. I realize that there have been violations in the content of the form, and I take full responsibility for this oversight. In light of this, I kindly request that the offending fields be removed from the form. Alternatively, if it is feasible, I would appreciate the removal of the entire form.

    I sincerely apologize for any inconvenience or disruption this may have caused. It was not my intention to violate any guidelines or policies. I assure you that I will be more vigilant in ensuring compliance in the future.

    Furthermore, I humbly request the reinstatement of my account by removing the form. I understand the importance of adhering to the rules and regulations set forth, and I am committed to upholding them going forward.
