Why have I been suspended?

    Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named New Customer Registration Form that’s breaking our Terms of Use by collecting login credentials.

    I've deleted the offending fields and reactivated your account. You should be able to log in now.
