How to embed the Contact Us form in Wordpress menu

Actually I wish to have the form in the menu point "contact us"

I have already a contact form there from jotform but it does not work

Why?

All other forms also from jot form ar ok, only this one contact us form does not work.

Can you please share to us the URL of your Wordpress website where you were trying to embed the Contact form. We need to check the actual form used on the website.
