I have a paid account, have four email addresses set up in the recipient emails box and the only the first email address is receiving the email. My email history only shows the email going to that email address and not the other three. Any ideas?

Can you please confirm first if this was your form that have the issue.
We will check further once we identified the correct form.
Please take note also the conditional email will override the settings of the Email templates.

So if the form was using conditions, it could be overriding the target Email Recipients of the original Email template.
We will wait for your updated response.
