Unable to locate the shared form template

I have previously shared a from as a template and deleted but now the shared forms don't appear in the templates. The prompt indicates the form has been saved to the templates.

Normally, the form template can be opened by using the form title. Here's an example:

As you can see, the form title is separated by "-" instead of space.

May I verify if you still remember the form title, form ID, or URL?
