My last name appears in the URL when I try to customize the link to the form

Hi, 

My last name appears in the URL when I try to customize the form. I'd rather it had our company name, not my last name. 

Are the jot form URL with the long number or the customized URL with my last name my only choices?

The only other option would be with the JotForm Enterprise account, you are able to create a Custom Domain for your URLs.
