How do i create a short cut for a form so the form appears on the phone

You can add a shortcut icon on the iPhone but on android, this is not possible currently.

You need to manually create the shortcut/icon with links to your mobile phone. Here's an example for iOS:

Or you can also let them install the Jotform Mobile app. Then just assign your forms to them so every time they open the app, the forms will be there for them to fill out.
