Allow users to select filling type and quantity for each product

I would like to design this form
Chicken with Filling
(And then Filling type and QTY)
Ie
Bacon & Cheese (Qty)
Ham Cheese (QTY)
Your help is valued

If I understand you correctly and based on the only form I can see on your account, you would like to sell some products and want to allow users to select the Filling Type and Quantity.

As per checking, your form is currently using regular Number fields. I suggest using the Product List field which has the capability to add options and quantity selector.
