Why is my uploaded file not showing up in the Preview mode?

Yes, but last page has an attachment of the HIPAA form, I believe page 5

which can be seen on the "BUILD" page, it does not seem to show when you

click on the link to the form. I am happy with all pages 1-4 then need the

HIPAA info included so that clients only have to click on the 1 link to get

both forms.

I have cloned your form from 202954496129161, and I could see the uploaded PDF in Build and Preview mode. Here's a screencast.

I have cleared your forms cache. Could you please clear your browser cache and try it again?

One question, do you want to upload this HIPAA PDF to allow your clients to do a download? Or, you also need to fill this PDF using the form?
